Jose Angel Vera, 33, fell during what police described as an aerial
acrobatic performance in which the man slowly unravels his body from a
sash while suspended in the air, Scottsdale police Sgt. Mark Clark said.

But Clark said "something went tragically wrong," and Vera fell on his head
in front of a crowd of about 700 at the opening dinner in the Bird's
Next party and concert tent at the FBR Open golf tournament.

Vera was unconscious after he hit the ground, according to a police report,
and off-duty emergency services personnel performed CPR until
Scottsdale Fire Department paramedics arrived and brought him to a
nearby hospital.

Acrobat Jose Angel Vera died while preforming a routine at an event in Scottsdale, Arizona. Vera, a 33 year old resident of San Deigo, died after falling from a height of about 15-20 feet head first. Although doctors who were in attendence rushed to his aid, he was pronounced dead at 7:30.

He died from falling and the stunt was a chain stunt.  The name of the stunt has not been told publicly, yet it looks as though he was climbing high trying to break free of chains or balance on the chain, and lost his balance to fall to the ground

According to the news article:
San Diego resident Jose Angel Vera, 33, was performing an acrobatic stunt involving chains in front of more than 700 people. As he was unraveling the chains, he fell from the rafters onto the ground. Witnesses said he fell on his head.

Jose Angel Vera was pronounced dead at 7:30pm on January 27, 2009. The acrobat was performing in front of an invitation-only audience of 700 at the Birds Nest at the FBR Open. The FBR Open is an annual week-long golf tournament held at the TPC Scottsdale.

Vera, 33, of San Diego, fell head first from a vertical chain raised about 15 to 20 feet above the crowd while he was performing at 7 p.m. on January 26, 2009. Although there were doctors in the audience who tried to help him, Vera was pronounced dead at 7:30 p.m.

Vera was one of five performers with the Aerial Artistry troupe.
