Essentially it's caused by three things:
1.Excess Oil Production
2.Build Up of Dead Skin Cells
3. Bacteria (P. Acnes)
It could be affected by diet for some people, but there's never been a scientific correlation established.
So you need to deal with those three issues.
1> For oil, cleansing with a gentle, effective clenser. Salicylic acid is a beta hydroxy acid that is oil soluble. This means it dissolves excess oil and will really clear out those pores. This can be in a cleanser or in a topical treatment that would stay on and control oil all day. Use as low of a percentage that you can get away with.
2> For dead skin cells, you need to be exfoliating regularily. Again something gentle. NO NUTS OR SHELL SCRUBS...even if it smells like apricot. Spherical beads, jojoba beads etc. Another great alternative would be Glycolic or AHA. They increase cell turn over and exfoliate quite deeply. You HAVE to use an SPF with these though as AHA's cause sunsensitivity.
3>Bacteria. Benzoyl peroxide is the best ingredient for this. It can be drying so only use it on the areas where the breakouts are the worst or as a spot treatment.
Even though oily is a problem for acne sufferes you have to hydrate. Look for oil-free, gel or emulsion hydrators.
I've worked with different acne products as an esthetician for the last 5 years, I also used to sell Proactiv. You have to be patient and consistant.

M$I don't know if there is a "best acne treatment," I would imagine that people's different skins and body systems would react differently to different medications, internal and external.
Never tried the fancy acne medications, but for minor acne, I find the over-the-counter benzoyl peroxide spot treatments work just fine.

these are some simple tips that you can do it at home...
-To prevent acne / breakouts due to blocked pores, use a mild soap-free cleanser to cleanse your face at least 2-3 times a day.
