Is there a way to tell someone he or she has bad breath without hurting his or her feelings? Maybe to leave an anonymous note at their desk?

Carry breath mints with you and, when you encounter someone with bad breath, take one out for yourself and offer the other person one. Make a light-hearted comment that's not directed at the other person, like, "I love these mints - they make my breath SO fresh!"
