A cold sore is an outward manifestation of the herpes simplex virus type one virus; that's the closest I can come to giving you the scientific name for a cold sore. The most common way to get herpes simplex is to kiss someone who has a cold sore.
Unfortunately, once you have herpes simplex, you will never get rid of it. However, you can treat cold sores with topical antiviral medications (Abreva or a prescription antiviral.) You can also take an oral antiviral medication which can reduce the amount of herpes simplex in your body, making it less likely that you'll get an outbreak and reducing the chance that you'll pass the virus to others.
Cold sores do go away on their own in seven to ten days, but again you will always have the virus in your body. You're not “better” just because the sore is gone.
Stress does cause breakouts of cold sores. Both physical stress (like a cold, hence the name) and mental anxiety or stress can bring them on. This is because when your body is stressed, your immune system is less effective. A less effective immune system allows the virus to build up in your body and cause the painful physical symptoms. For that reason, it is impossible to say how often you will get a cold sore.
