I smell mildew. I can't figure out where it's coming from but it's driving me INSANE! What causes this?

A damp environment. Other than that it's just spores in the air that start growing once they find the right place to start. A cool, dark, damp location will almost always get you mildew at some point.
A dehumidifier will help. Also a air cleaner will help remove the spores from the air.
