I have a good bit of experience with this cloud database solution and this should be simple and easy for you to set up. Just identify the fields of information you want to collect. Set up a a record layout with categories that correspond to those types of information and create a public form view.
To get more details about what you need to do just e-mail the Dabble DB support team. The respond fairly quickly and are very helpful. I am sure they can help you with this. Also the service is cheap ($8 a month) and they will host the data and provide you a public link.
You will not have to do anything overly complicated. Just take the url from the form and put it on your webpage and it will redirect to form they host for you.

M$That being said, there are a few form sites that you can find that will do exactly what you want, and many of them are free. I can't personally endorse any of these, as I've never used them, so make sure to take a good look at them before you actually use it.
Wufoo looks pretty clean and professional
Bravenet has been around a while and has a lot of functionality, but I've never particularly cared for their interface.
FormSpring is one other one I found that looks pretty professional as well.
