about Anal Fissure

An anal fissure is a tear in the skin of the anus. It is very painful especially when it is acute (new). Under normal circumstances a small would like this anywhere else on the body would heal with out much fuss. The reason why anal fissures don't heal has to do with high pressure and spasm of the underlying sphincter muscle . This is the muscle that prevents you from leaking stool. The problem is that when this muscle squeezes there is inadequate blood supply to allow the would to heal.
The first line of treatment for fissures therefore are aimed at measures to relax this muscle naturally. They consist of hot baths and increasing dietary fiber to increase the diameter of stools to allow this muscle to relax. Ointments can be applied to the area to help the muscle to relax (nitrates) but interestingly they give most patients a severe headache.
If this treatment doesn't work the muscle can be relaxed by an injection of botox. (insert joke about helping wiith anal wrinkles here).
Surgey is the next option. The surgery for this involves partially cutting the muscle to allow the would to heal. This works nicely but unfortunately about 1-2 percent of patients will have some degree of incontinence after this procedure making this something to be carefully considered.
