Actually the term is not used appropriately. Grand Slam is referred to wining the four major Tennis tournaments in the world in the same year. They are:

* US Open
* Wimbledon
* Roland Garros
* Australian Open

Today, people usually refer to Grand Slam by winning any of those tournaments at any given moment but that is a misconception. In reality they should be referred simply as a Slam Tournament... if you win the four of them in the same year, then that will be a Grand Slam. If a player wins all four consecutively, but not in the same calendar year, it is called a Non-Calendar Year Grand Slam, and winning all four at some point in a career, even if not consecutively, is referred to as a Career Grand Slam.

So, we can say now that Roger Federer has won 16 major Career Grand Slam. The last man to win the Grand Slam I believe it was Rod Laver in 1969. For women it was Steffi Graf in 1988, she also won the Olympic Gold Medal that same year at Seoul Summer Olympics, for that reason it is called Golden Slam. Graff is the only person in Tennis history to achieve such a feat.
