How does a muscle relaxer know which muscle to relax? Hmm....

Muscle relaxants work by affecting the nervous system, either centrally or peripherally, so all muscles are affected. The effect appears most significant in the target muscle because the localized pain relief (the spasm is also caused by signals from the brain which are affected by the muscle relaxant) is more noticeable than the slight generalized loss of muscle tone caused by the medication.
