Place portals like shown in the first photo. Jump into the blue portal to come out of the yellow one, and then aim to land in the blue portal again. This will shoot you up to the top platform.
Now place portals like shown in the second photo. Jump into the blue portal and you should have enough momentum when you come out of the yellow portal to make it to the platform with the exit.
