It's different for different cars. Some cars even have devices that make it very difficult to do so. A slim jim works best, but if all you have is a coathanger, you still may be able to get it open.
Get yourself a nice, stiff coat hanger. The stiffer the better. Flatten out the coat hanger, but leave the hook on the end. Slide the coat hanger down in between the window and door somewhere in the middle of the door with the hook facing toward the door handle. Once you get it in a good long way, approximately a foot or more. Slide the hanger toward the door handle. When you get to the door handle, pull up.
Note: This is very difficult to do and it may take dozens of times to actually get it. Also, like I said above, different cars are different so experiment by "pulling up" in different areas.

NOTE: You want to be careful when pulling the hanger around so you don't mess up the rubber surround of your window. Also, newer cars tend to have bars and blocks inside to make this more difficult. It will take a LOT of patience until you get the hang of it.
Also, yes the cell-phone car unlock wouldn't work. The remote unlockers actually work by sending radio signals, not sound which a cell phone would carry....

Anyone know how to unlock a car door (wire hanger or otherwise)? The remote-cellphone myth has been busted.

Basically you use the hook at the end of the hangar to snack the locking mechanism inside the door. It may be situated in different places depending on what type of lock you have (the little button you click sideways, or the straight up-and-down plunger). Snag the pin and pull to unlock... To save yourself from damaging your car, if you have a service like the auto club, call them, they will call a locksmith and most or all of the locksmith call will be covered or call a locksmith directly.
