I dont know how to rate accidents .
If one consider casualties here they go
3 worst submarine disasters are

1.April 10 , 1963 : USS tresher sank with all its 129 crew men. ironically tresher sank off new - england while performing safety test calle SUBSAFE to determine the safety of subs under the deep depths . Later its concluded that probably Tresher imploded at a depth some where between 1300 to 2000 feet (400 to 600 mts) and the disaster is due to faulty design / structure
2.Aug 12 ,2000:A Russian Oscar 2 class cruise missile submarine exploded in the barent sea killing 118 people on board while performing firing exercise of dummy torpedoes . probably due to rusty casing of torpedoes which couldnt hold highly concentrated hydrogen peroxide which is used as propellent in torpedoes.
3.May - June , 1968 : USS Scorpion disappeared in atlantic sea norfolk ,virginia killing 99 of its crewmen . Though various theories floating around on how this submarine sank based on wreckage actual reason for the accident still is unknown and inconclusive .
