Well, some actually do tweak your sense of taste a little while you're on them. The other thing is just that they're medicine, most chemicals taste astringent. So either you've got that, or else you add fake flavors to them to improve it and sometimes that artificial flavor adds an unpleasantness on its own.

http://www.netwellness.org/question.cfm/71321.htm mentions a specific antibiotic known for the aftertaste.
