According to the Society for Neuroscience

Neurological illnesses strike more than 50 million people annually, and cost more than $460 billion.
Mental disorders strike 44 million people at a cost of $148 billion. Delaying the onset of Alzheimer’s disease by five years alone could save $50 billion in annual health care costs.

May 7 - President Obama released his detailed FY2010 budget, which included $30.996 billion for NIH and $7 billion for NSF.

May 4- SfN publishes updated state fact sheets, which use state-specific data to illustrate the potential impact of research funded by NIH.

The main clinical issues are
- Neurodegenerative disorders (Parkinson's and Alzheimer's)
- Stroke (closely related to vascular diseases)
- Spinal cord injury
- Dementia (in all its forms)
- Addiction

It should also be noticed that Brain Cancer and Blindness are also tackled by Neurosciences.

5.Amyotrophic Lateral Sclerosis (in some cases it affects the brain and memory)

Not in any particular order. Not all are diseases. Some are disorders, or serious head injuriy is an accident, for example.

I won't venture a guess on the $
I don't know how someone would calculate the dollars spent a year.... worldwide or just in one country...

These are the 5 major brain diseases in my opinion. I am not sure about the total cost needed for the treatment
