People often fall for care givers. (IE doctors and nurses) The comfort they feel by being taken care of by these people can often lead to deeper feelings.

Be careful this happens to people in the medical field all the time and while caregivers often care about the people they treat it may not be a romantic feeling for them.

No, it is unethical for someone to be romantic with a customer. If you are no longer seeing him for medical reasons you can ask if he wants to go out for coffee sometime, but while you are under his care I would say to put your feelings aside
