The formula used to do so can be found here:

It is BMI = weight in pounds x 703 / height in inches squared

Your BMI is your weight in pounds divided by your height in inches squared, times 703 BMI =W/H^2 *703

You can use the Body Mass Index calculator from the National Heart Lung and Blood institute. It calculates your BMI based off of your height and weight. http://www.nhlbisupport.com/bmi/bminojs.htm
You can also calculate your BMI manually at:

BMI is basically weight divided by height squared all multiplied times a which compensates for the units used in the weight and height factor.
Be aware tho that BMI calculation is notoriously inaccurate when describing actual health. This is because it doesn't take into account the persons build. In other words if your stocky(short but wide) you will score too high. If your extremely athletic or in other words have a lot of muscle you will score too high. If you are extremely sedentary you will score too low as you will have almost no muscle mass.
If you want a good indicator of health you are better off getting measured and finding out your total body fat. And don't think that those little scales that run electric through you are good either. They can be precise(IE if you measure over time they can tell you a difference in losses or gains.) but they are not very accurate. The best way is to do the submersion tank but getting calipered can be very accurate too if done by somebody who is good at it.
Good Luck!

If you use the metric system, BMI is equal to your weight in kilograms, divided by the square of your height in metres. If you prefer feet and pounds, BMI is equal to your weight in pounds, multiplied by 4.88, divided by the square of your height in feet.
If you prefer inches, BMI is equal to your weight in pounds, multiplied by 703, divided by the square of your height in inches.
If your BMI is under 16.5, you are severely underweight.
16.5 to 18.5 means you are underweight.
18.5 to 25 is healthy.
25 to 30 is overweight.
Anything over 30 is obese.
BMI Prime is equal to your BMI divided by 25.
