I'm so sorry this question has gone for so long unanswered. I feel like congratulating you for stumping us!
I may have found a forum that can answer your question, however. www.Tdpri.com/forum provided a wiring diagram for a similar question, "Is a 4 way switch and 2 push/pulls for tap and phase possible?" The link appears in "sources" below.
I think if anyone can give you a solution, they'll be the ones. Good luck with modding the Stratocaster!

Is it possible to wire a Humbucker pickup to get both "out of phase" and "coil tap" on 2 push pull switch?
I got a Fender Stratocaster with a H-S-S pickups, 1 Volume and 1 Tone configuration, 5 way switch.
I've been searching the web if its possible to get BOTH "out-phase" and "single coil" option on the Bridge Humbucker. So far I haven't found anyone who have done this yet.
Could it be done if I install 2 push-pull pots for Volume (out-of-phase) and Tone (single coil). How do you wire it (schematics pls.)?
