I need for my daughter to find the guitar chords of the Shushybye Lullaby (details inside)

Hi,
The song called "Shyshybye and Goodnight" can be heard in this youtube clip, beginning at 4:49
I need the guitar chords of this song, not the other ones.

This transcription will be easiest and is mostly accurate. Sorry, I tried to make it easier to read but Mahalo deleted the extra spaces. Pretty involved verse for a kids show.

Song is in G (tonic)

There are 4 chords per line in my chart.

Also, ignore "Double Time" next to "Verse" - I thought I took that out but it is still there and I can't edit it.
