Nintendo has done the best job. The WII encourages physical exercise while the DS has software for mental chalanges, and they have software on healthy recipes, etc. (Sorry... I answered before reading your description). http://shopping.yahoo.com/p:Health%20&%20Fitness%20Club%20Tycoon%20PC%20Game:1991553294

Not sure where you are going with this one. If you want to exercise, I'm thinking the only games are WiiFit or perhaps Dance, Dance Revolution which is available for multiple consoles and requires a dance pad.
If you are thinking about health of other people, there is a game called free rice where you can play a game of matching words to their definitions. Doing this donates rice to developing countries.
Or there are programs for donating CPU cycles of your computers to help solve equations dealing with different health topics. The most famous is folding@home. I think the other one is world community grid by IBM. There are undoubtably others.
Perhaps you could refine your question for more direct results? (I'm not being mean here. I'm just trying to help focus the question for better results.)
If you are looking for a browser based game that gets you to exercise, I'm not aware of anything like that.
