It's both ... in a way.
The typical mouthwash contains the active ingredient, ethanol, or otherwise called alcohol. It does contain actual alcohol content, so it is possible to fail a breathalyzer test after using mouthwash. Though it has alcohol, it is not meant to be swallow, so don't worry. As long as you don't swallow, you will not be under the influence.

A report Dental Journal of Australia claims that there is "sufficient evidence" that "alcohol-containing mouthwashes contribute to the increased risk of development of oral cancer". Yinka Ebo of Cancer Research UK, however, says that "there is still not enough evidence to suggest that using mouthwash that contains alcohol will increase the risk of mouth cancer". The claim is still being debated today.

Bottom line is, try the alcohol-free mouthwashes. You can continue cleansing your mouth free of potential disease-causing bacteria without worrying about whether or not alcohol will cause mouth cancer.
