The only time I would use a spoon and fork together would probably be when I would eat pasta or rice dishes.
Forks
The fork (from Latin "furca" which translates as "pitchfork") was used as far back as the ancient Greeks where they used a fork as a serving utensil.
The fork is also referred to in the Hebrew Bible, in the Book of I Samuel 2:13 ("The custom of the priests with the people was that when any man offered sacrifice, the priest’s servant came, while the fresh flesh was boiling, with a fork of three teeth in his hand...").
It became a general custom after the 10th century in Western Europe to use a fork for eating meals and continued to expand across the world.
Of course, the "chopstick" was used in the Shang Dynasty (1600-1100 BC) in East Asia in places like China, probably one of the first "eating" tools to exist.

As time progressed through the middle ages, the shape of the spoon changed to that of a spoon we would use today to eat food like soup or cereals.
There are lots of references to spoons in history. Northern Europe references state they used a small "chip" of wood. The Ancient Greeks used "shells" to scoop food into. Even the Ancient Egyptians had small "slates" where they would shovel grain or other food.

I agree with cjd that is the only way I have seen people use a spoon and a fork is to eat spaghetti. They pick up spaghetti and put in spoon to twill around their fork!

I've heard of a "spaghetti fork" in which there is a handle which can rotate around the fork part.
