I don't usually go as far as asking how things are prepared, but I do try to be mindful of what I'm ordering, and 'try' to choose things that are better for me than others.
Sometimes I eat what I want, but I don't always order that way... Usually I try to order grilled chicken, rather than beef, and I try to order things that would have vegetables or other fresh ingredients in them.
I also try to opt for rice dishes, over pasta dishes. It just seems to me that eating a meal with rice on the side would be much better for you than the same main ingredients smothered in some sort of heavy sauce, over a pile of noodles.
But I must say too, we have an amazing Thai restaurant near our house, and I *hope* their food is as healthy as I assume, because it's fantastic and I can't help but help myself to more, when we go there!

When dining out, do you try to "eat healthy" by making special requests or asking how items are prepared?

I certainly used to do this when I was on Atkins. Now, I basically eat what I want but watch my portions, so no, I don't do this anymore.
