Of ALL TIME?
My vote for SCARIEST is prolly The Shining. That movie still freaks people out who have never seen it - that's powerful. Shoot, it freaks me out and I've seen it 100 times!!
My vote for Goriest has got to be the newer version of Chainsaw Massacre however, I also feel that the old school Friday the 13th movies were quite bloody.
As far as disturbing goes? I'm all in on The Devil's Rejects. The beat down in the middle of that flick is...well....qite disturbing.

One of my aunts saw "Psycho" when she was a teen and refused to shower for WEEKS unless my Mom (her kid sister) was standing outside the closed door. Even though they lived in a one-family house, and there wasn't any window in the bathroom for a bad guy to sneak in through.
Then my Mom saw "The Birds" and freaked the next morning, on her way to school, when she saw a large flock of crows gathered in the field across the street from their house.

The only movies that have ever REALLY scared me were directed by Alfred Hitchcock. Especially "Psycho" and "The Birds." The beautiful thing about his films was that he used your own imagination against you, versus using a bunch of special effects to make the boogeymen for you.
