Those are the big questions that the series has yet to answer.
What do I think?
No, it's not just all in John Locke's mind. And I don't think the Island, proper, is intelligent. But I think that there are supernatural entities on that Island that project those images to Locke and others in order to manipulate the crash survivors.
I think the supernatural entities are protecting....I have no clue. But whatever it is, I think it's a battle of good vs. evil.

What do I think?

1. Locke imaged the island could communicate with him, a beautiful woman
2. Locke imaged he could walk again, fantasy
3. Locke imaged that characters on the plane were stranded on his Island
4. Locke says, "the Island gives you what you want, but it must be given sacrifices". Many individuals are being sacrificed, drama
5. Locke believes he is the Messiah
6. Locke will power to carry Boone after the accident proves he strength in his dreams are the result of his will power.
7. The monsters don't hurt Locke.
8. Locke said, "it was a test"
