Spoiler!
I beat the game following the good path and got into Project Purity along with Sarah (I think) Lyons. When I was told someone had to go in and get irradiated to start the machine, I was given the choice of going in myself or sending in Lyons.
If you send her, you have to cycle the air lock to send her in, and you get a slightly tweaked ending in which Ron Perlman says you couldn't live up to your father's shining example and let a real hero sacrifice herself. You live, but there's no more information about what happens to the Vault Dweller.
Seems like you should be able to send Fawkes in on the good path too, since he's a Super Mutant and immune to radiation. When I volunteered to go myself, then asked Fawkes to go in my place, he refused to rob me of my "destiny."
Either way, the ending was basically the same, except I was essentially belittled in the ending for being a coward.

In Fallout 3, is there a way to live through the ending?
I found 3 endings, died in all of them. Is there a way to live through the ending?
