In the episode "Doomsday" - Rose stays with the Doctor to help him close the breach. They hold two magnetic clamps that hold them against the wall - whilst the Cybermen and Daleks are pulled into the Void. However, Rose's lever slips and she pushes it back into the position but as a result, falls into the Void. Then Pete (her father) catches her and transports back into his world. Minutes later, the breach begins to close, leaving the Daleks and Cybermen trapped in the Void and Rose trapped on Pete's world, on a parallel universe.
The Doctor couldn't open the breach otherwise the Daleks and Cybermen would travel into Earth once more.
Rose returns in Series 4 - with her brief appearances in "Partners in Crime", "The Poison Sky" and "Midnight". However, she prominently featured in "Turn Left", "The Stolen Earth" and "Journey's End".
In Turn Left, she has somehow managed to cross reality to find the Doctor - however finding out that he had died.
In The Stolen Earth, she returns again - because the walls of reality are breaking down. In Journey's End, Rose returns to the parallel world with the Meta-Crisis Tenth Doctor.
However, it looks nearly certain that Billie Piper will play Rose Tyler again to end the Tenth Doctor's Reign in the 2009 Specials.

This episode also hailed back to the 'Rise of the Cybermen' episode, in which Rose's alternate reality father and Mickey were fighting the Cybermen. The Cybermen were nearly defeated in the alternate reality and they followed them to Rose's.
The ghosts were actually Cybermen and the void ship opened and revealed daleks. The human's from the parallel earth have developed technology to jump back and forth between earths. Each transfer between worlds is is causing damage to both worlds and if the access to the void is not closed then both worlds will fall into the void.
At the end of the episode the doctor is trying to get rid of the daleks and cybermen by sucking them into the void. Rose tries to help him but starts to get sucked into the void. Her father materializes at the last minute and pulls her into his world, saving her from the void. Once the entrance to the void closes, she cannot get back into her earth and is permanently separated from the doctor.

Rose getting stuck in a parallel dimension is in the 'Doomsday' episode of series two. This is part of a two part episode which started with 'Army of Ghosts'. At the beginning of the tale, ghosts were appearing around the world, this was related to a Torchwood institute that was conducting experiments. The Torchwood building was built to reach an anomaly that the ghosts and a void ship came through. The experiments were increasing the breach between worlds.
