On this one, I'm not sure I've got the story right, but this is what I think happened.
1) How did the Doctor escape Sutekh and trap the destroyer?
As I understand it, he didn't exactly escape. Sutekh forced the Doctor to go to Mars and disable the force located there that kept him trapped. When Sutekh was freed from his prison he started moving down the spacetime tunnel, leaving the Doctor and Sarah-Jane where they were.
The Doctor was able to trap him because using the TARDIS he was able to reach the other end of the tunnel first, and reprogram it. It sounds like the idea was that either Sutekh or the signal from Mars to control the tunnel could only move at a lmited speed and woud take two minutes to reach the Earth, so the TARDIS could get there first. (I'm not too clear about that part!)
Anyway, using some equipment from the TARDIS, the Doctor was able to reset the spacetime tunnel so it stretched for thousands of years, far longer than the 7,000 year lifespan of an Osirian like Sutekh. Hence Sutekh was trapped in the tunnel and assumed to have died there.
2) Who built the Pyramid of Mars that traps Sutekh?
Presumably it was Sutekh's brother, Horus, together with the other remaining Osirians who were helping him. I don't know that it's stated explicitly, but it's hard to see how it could be anyone else.
3) Who activated the Eye of Horus that immobilizes Sutekh on his throne chair?
Presumably that was Horus too.
4) Did Marcus Scarman accidently open the time tunnel?
As I understand it, Marcus Scarman did not open the tunnel. He stumbled on the tomb where Sutekh was trapped, and Sutekh killed him. Sutekh took over his corpse and then activiated the tunnel to send Scarman back to his home. where he would be used to build a rocket to destroy the Pyramid on Mars which kept Sutekh himself imprisoned.
5) Did Sutekh return in future episodes?
Sutekh never re-appeared in the TV series. (At least as yet!) However it seems, he did feature in a couple of audio CDs in the "Faction Paradox" series.
