does this preclude me from applying for a job as a probation officer

It doesn't seem to mention any preclusion based on past medical history. As far as I am aware during the normal course of a job application an employer does not see your medical history. I'm not even sure it is legal for them to, and if they did, they would definitely need your permission. I would say if you didn't mention it, you have a nearly 100% chance of it not applying.

Not at all, I have been dealing with depression since i was 11 years old. I have been hospitalized several times for suicide attempts, and spent weeks to months on the mental health ward.

An employer cannot look into your health records without your written consent, and if they made a decision not to hire you based on a medical background that would be descrimination against you.

So, in conclusion, don't say anything about it, it is not a disability since the reason for hospitalization and depression was caused by the breakup.
