My question is, what features are you looking for in a contact form for your own webpage? To get the M$1, Please be as detailed as possible.

from, email, name, address, telephone, preferred method of contact (email, phone, hours to call back, time zone)
date subject hour and time zone for when message was sent
question, message space
suggestions for improvement to the business website by the message sender!

Yes I have made a forum by form genarator.I inviteed my facebook and twitter friends to submit here.Now I am still developing it.

You need the 'message' field, where people can write any message they want! So far it only lets you choose recipient and subject (I think)?

The message field appears. This is one version of what the contact form can look like when you're done generating it:

CC and BCC fields are good ideas. Thanks. I'm not sure what you mean about a field for FROM.
A From Field would allow them to Change what is displayed as the sender in their Reception box, ie., FROM: Answers@mahalo.com
The FROM field already exists for entering an email address. It is useful for avoiding spam as well.
The information I like to see in a contact for is as follows
Name
Contact Information (phone, email, etc)
General Notes section
Ability to attach a file(s)
Captcha (to reduce SPAM)
Name
