What are the leading causes of death due to CVD?

I am trying to find a list of the top CVD causes of death - So far I can find:
1. Heart Attck
2. Stroke
3. DVT (please confirm these are correct and/or help me find the longer list -

From what I found, it is:
1. Heart Attack
2. Stroke
3. Heart Failure
4. High Blood Pressure
5. Tobacco
6. High Blood Cholesterol
