Not having her own opinion or thoughts on anything...

I'll say it right up front, I like a mental challenge. And I like to go wherever the conversation flies...but I don't care how gorgeous she is...if all she can say is, "Oh mah Gawd! I was in the store and saw the cutest little..." bleh...

although you didn't ask it...one of the bigger turn-ONs is actually when she will spar back and forth with me on a subject. I really don't care what about...science, philosophy, history, sociology, or just something stupid. If she will challenge/overturn my thoughts/opinions with intelligent ideas of her own...Ooooo....

I think the biggest thing for me, at least on a first date, is answering your cell phone. Some times it's an important call, but recently I had a girl answer her phone while we were having dinner and talk to her friend about nothing for around 10 minutes. I can't tell you how much of a turnoff it was to be sitting there fiddling with the straw in my drink while she was yacking to her friend. I was really into her until that happened and because of it I never called her back for a second date.

Cell phone usage would be a huge turnoff. A date is a way for two people to get to know one another. If someone carries on a random conversation while on a date...I would assume they are not that interested in getting to know me. Nice answer!

When the girl constantly talks about herself, and doesn't ask a single question about you or your life/interests.

It's a bad sign when you come home from a date and realize that the person you just went out with doesn't know a single thing about you, and you know ENTIRELY too much about them.

Oh, it's difficult for me to pick my *biggest* dating turnoff. There are so many! One of my major picks would definitely be when a guy is rude to waitstaff. Regardless of how gentlemanly a man acts towards me, if he is rude to the waiter, he loses my respect. It really shows his true colors.
