I suggest you look on Facebook or Myspace for him. Then just shoot him an email saying "Hey, how's it going?" and maybe a little of what you have been up to. You're also likely to find out if he is in a relationship from his profile status in case you don't know. If that doesn't work, look on classmates.com and other social networking sites. He's likely on one of them.

An ex girlfriend once contacted me after over 15 years, so I bet it's easier than you might think. Maybe an email if you are nervous about face to face?or a text? A simple "hi" won't be the end of the world for either one of you.

You don't say if you want to get back together with him? Start slow, don't
open with that. Spend more time catching up first.

Just pick up the phone and call him. Tell him he was on your mind and you just wanted to say hi! If he missed you also conversation will just come natural! Go for it, what have you got to lose?
