After doing a little bit of research, I believe that you are able to transfer the personalized plates, provided you are a resident of NYC. If you plan to purchase the vanity plate in NYC and register it in a different state, as @knightofvalar said, can be a problem. To be honest, I do not think that you will be able to, because you will have to find out first on the different state (should this be the case) whether or not, that vanity plate already exist and registered there.
To help you find more accurate information, I suggest you contact the official DMV (Department Of Motor Vehicles) of the state of NY. Here are some of the FAQs regarding personalized (custom or vanity) plates:

If you do live there, however, then you should go down to a local DMV office with the owner of the personalized plates and you might be able to transfer them there.
They allow one person to transfer these kinds of plates to another vehicle they purchased, so long as the registration class did not change. So, if the new vehicle you will put the vanity plates on is the same as the one they used to be on, what you may have to do is have the other owner return the plates to DMV, then you buy it from them.
But it depends on the DMV office as I read their rules on vanity plates varies.

So there is provision in the forms to transfer plates as well as ownership of the car. There are forms to surrender plates if that is needed for the old ones.

Can I transfer a vanity (personalized) plate to another person in NY state?
I want to buy someone's personalized vanity licence plate. They live in New York State. I know some states allow you to buy/sell vanity plates. Can I transfer a vanity plate to me from someone else? Is there a process for this?

New York state does not allow vanity plates to be taken out of state, so if you do not also live in NY, there would be a problem. If you do live there, however, then you should go down to a local DMV office with the owner of the personalized plates and you might be able to transfer them there.
They allow one person to transfer these kinds of plates to another vehicle they purchased, so long as the registration class did not change. So, if the new vehicle you will put the vanity plates on is the same as the one they used to be on, what you may have to do is have the other owner return the plates to DMV, then you buy it from them.
But it depends on the DMV office as I read their rules on vanity plates varies.

Hi, the FAQ on the site only refers to the situation where you already own a personalized plate and you buy a new car and want to move the plate from your old car to your new car.
It doesn't cover the idea of moving your plate to a different person.
That is what I need to know about.

But it that situation YOU are the 'other person' - this is what the person who is selling the plates to you should do. You are the buyer yes? They are the seller. In this case they fill out forms and so do you. I'm sorry if I didn't make that clear. You can explain this to your seller if they don't know the right thing to do at the DMV.
Generally, when you buy and sell cars and parts and plates, both seller and buyer are required to complete forms. In this case, they complete a different form than they would otherwise do.

And I'm assuming you live in NY as first poster has said they don't allow those transfers out of state. But if you do, its the seller who needs to complete other sections to transfer ownership of the plate to you.
