Yeah I have a few times, most especially if you were discussing something else then you wanted to raise a question, but your supervisor didn't seem to notice, or simply wanted to have questions asked after the end of discussion.
But then they already jumped into a different topic and you got too engrossed in the new topic just the same.
When your superior starts entertaining views, questions and opinions and calls you to voice out what you have to say, only for you to realize that you have forgotten what you wanted to ask.
When this happens I just smile and say, it's already been answered in the discussion, and signals them to continue...
Then I start thinking what was that again that I wanted to ask, then just smile to myself. (lol)

This also happened to my friend once and then he just scratched his head and said, if you'll excuse me I have to get out of the room... (lol) Then we both ended up laughing after the meeting.

Yes I have and the situation is so embarrassing as you scramble to find the thought you just had while trying not to stammer and let everyone know. Usually you have to just admit. "I lost it, I lost the answer" and laugh.
