The winner of the ACC tournament, which has been held since 1954, wins an automatic birth in the NCAA tournament. In addition, the ACC virtually always wins a handful of the 34 at-large bids to the 65-team NCAA tournament. In 2009, for instance, the ACC sent seven teams to the NCAA, including the eventual winner (the University of North Carolina). http://ncaatournament.fanhouse.com/viewbracket The 2009 champ was Duke http://www.theacc.com/championships/09-mbk-tournament.html, which followed two consecutive titles by North Carolina. Duke has been the most successful team in the ACC tournament over the past decade, winning eight of the last 11 tournaments.
2008 was the 25th anniversary of NC State's conference championship victory, which gained them entrance into the NCAA Tournament. The Wolfpack, led by head coach Jimmy Valvano, made one the great surprise runs in NCAA Tournament history, winning the national championship on a last second dunk by Lorenzo Charles.

In 2010, the top seed in tournament will be Duke. Other hopefuls include Maryland, Virginia Tech, and Wake Forest. The only remaining teams that are above .500 are Clemson and Florida State. Notably, defending national champ is one of the conferences worst squads, well below .500 and with little hope of making the NCAA.
